We conducted a survey to find out what caused these nursing-related mishaps. The subjects were 881 nurses working in 5 different institutions. The nursing actions and processes involved in the incidents were: collecting and assessing patient information, nursing planning, implementing nursing plans, and evaluating results. It appears that the psychological state of the individual nurse has a determining role, together with inter-personal relationships and communications, in the development of these incidents.
