Methods : We conducted semi-structured interviews involving one nurse manager, one nurse and one social worker who work at health services facility for the elderly in a Metropolitan area. This facility works on support for in-home care actively. Results : The nurse manager and the nurse shared about users' information about their physical condition and care methods with health care workers in the community through the social worker. The social worker cooperated with care managers, focus on continuity of the health care plan of the user.
