【目的】介護老人保健施設（老健）利用者の在宅支援に必要な看護職および相談職のケアおよび連携の内容を明らかにする．【方法】調査対象は都内にある在宅支援を積極的に行っている老健１施設の看護管理者，療養棟看護師，相談職管理者で、「在宅支援に関するケアの内容と意図および取り組み」について半構成的面接を行い，逐語録を作成して内容分析を行った．【結果および考察】今回，看護管理者，療養棟看護職が相談職を通じて，地域のケア職と利用者の状態やケア方法について情報を共有し，相談職管理者が施設と在宅のケアプランの連続性と利用者の健康管理を意図して，ケアマネジャーに働きかけていた．こうした連携方法の背景には，相談職管理者がケアマネジャーの資格と共に看護職としての職歴を持ち，利用者の健康状態のアセスメント能力があったこと，在宅生活の継続には健康管理が重要であると捉えていたことが考えられた． |

Aim : The aim of this paper is to clarify the function of health services facility for the elderly from view of support for in-home care. Methods : We conducted semi-structured interviews involving one nurse manager, one nurse and one social worker who work at health services facility for the elderly in a Metropolitan area. This facility works on support for in-home care actively. Results : The nurse manager and the nurse shared about users' information about their physical condition and care methods with health care workers in the community through the social worker. The social worker cooperated with care managers, focus on continuity of the health care plan of the user. Conclusions : In the background of such cooperation methods, the social worker has an employment record of the nurse and the ability to assess a user's physical condition, so that she thinks health management is worth continuing for the home life of the users. |
