| 内容記述 | 介護老人保健施設の終末期ケアにおける看護管理者の役割を抽出する目的で、施設特性や終末期ケアへの取り組み開始時期が異なる３施設の看護管理者３名と看護師１名に、半構成的面接を行い、管理者の終末期ケアへの認識や取り組み、困難を感じている点とそれを解決するための工夫を分析した。 老健の看護管理者は、施設特性や終末期ケアの経験・認識の違いによって異なる役割を果たしていた。管理者はこうした影響を認識し、それらの違いを超えて、高齢者・家族が意思決定できる仕組みを作り、症状緩和の処置と高齢者の苦痛のバランスの調整、日常生活重視のために、医師・他職種・家族と調整を行い、連携に必要な介護職の能力向上のための教育を行う必要がある。 |

| 内容記述 | In order to deduce the role of nurse managers in end-of-life care at Geriatric Health Service Facilities, semi-structural interviews were conducted with nurse managers and nurse at three facilities which had different characteristics and which began to tackle end-of-life care at different periods. An analysis was made of these managers' perceptions of end-of-life care and their ways of tackling it, and of aspects about which they felt misgivings and how they set about finding solutions to these. Nurse managers in Geriatric Health Service Facilities performed different roles depending on the characteristics of the facilities and their experience and perceptions of end-of-life care. There is a need for managers to perceive such influences and go beyond the differences in order to build a mechanism that enables elderly persons and families to make decisions; to adjust the balance between symptom relief procedures and elderly patients' suffering; to liaise among doctors, other personnel and families in order to emphasize daily life; and to provide education essential for improving care-givers' ability at liaison. |
